The Function-Focused Stabilization Program is a structured, referral-based service designed to support patients with complex medical, behavioural, and medication-related needs.
Care is provided through a consultation and time-limited stabilization model. The program emphasizes function over symptom-only care, safety in prescribing decisions, clear communication with referring physicians, and a structured return-to-primary-care plan.
Every care plan is individualized based on functional ability, safety risk, and behavioural or coping patterns affecting adherence and recovery.
A referral is received with a clear clinical question and relevant background information.
Full medical, medication, function, safety, and behavioural pattern assessment.
Structured follow-up, medication optimization, taper planning, OAT care, or functional recovery pathway as appropriate.
Clear care plan and monitoring guidance are provided back to the referring physician.
This program is not a walk-in clinic, not a refill clinic, and not a replacement for primary care. It is a structured stabilization pathway designed to clarify risk, improve function, and support shared-care planning.
Appropriate referrals include complex opioid or benzodiazepine use, polypharmacy concerns, OUD or substance use, chronic pain with functional impairment, and mental health concerns affecting function.
This program is designed to help patients with complex medication use, mental health concerns, chronic pain, addiction, or functional symptoms improve daily function and safety.
The goal is not simply to reduce medications. The goal is to ensure treatment is safe, effective, and helping you live better.
